General information |
 |
| The project leader (or a designated person) must meet with laboratory
management well in advance of the start of the project. Please make
an appointment with laboratory management (call 3-3255) to discuss
the research project as soon as you have any project details. An X-Customer
Account Form can be filled out in advance of meeting with laboratory
management. However, it may be easier to complete it during or after
this meeting. Research projects will not be performed by our laboratory
until this meeting has taken place (regardless of whether the form
is filled out or not). Project requests and scheduling may need to
be changed depending on the ability of the laboratory to handle the
number of samples and testing required. |

X-Customer Account Form |
 |
| This form is downloadable in PDF format (you will require either
Adobe Acrobat or Adobe Acrobat Reader to view this file). The research
account name must be limited to 25 characters. After the form is completed
and the project is approved, paperwork will be returned to you, stating
an x-customer account name and computer number. This information,
exactly as stated, must be on all laboratory request forms. |

Specimen Labeling |
 |
Animal ID: All specimens must be labeled with an animal ID. Please
use only letters and numbers. Do not use any other symbols (-,/,#,).
Keep a list of the I.D.'s submitted. Be consistent & use EXACTLY
the same I.D. each time that animal is sampled. If serial samples
from the same animal are submitted, a cumulative report of results
will occur ONLY if the I.D. of the animal is EXACTLY the same for
each request. |

Request forms: a laboratory request form must be submitted with all
samples. Laboratory request forms are available at various spots throughout
the Cornell University Hospital for Animals and in the Clinical Pathology
receiving area. Request forms can be completed when the samples are
submitted or a supply of request forms can be taken ahead of time
to complete at the time of collection. Multiple samples, having the
same test(s) performed, can be listed on ONE request form, however,
each sample must be clearly identified. Request forms MUST include: |
Patient I.D. (limit of 10 characters)
X-customer account name and computer # (see above)
Species
Researcher's name
Date & time sample(s) taken
Test(s) requested

Timed samples (e.g. glucose samples drawn at 8am, noon, 4pm over the
course of a day): researchers should be sure that each sample is labeled
with the DATE & TIME and the request form shows the same DATE
& TIME. |

Time of submission: Samples should be submitted as early as possible
in the day. This will result in timely reporting of results and allow
better distribution of the work load. Do not submit samples late in
the day (after 3.00 pm). |

Research samples are NOT accepted on SATURDAY or during the HOLIDAYS,
unless specific arrangements have been made. |

Reports |
 |
| Written reports of laboratory results are printed during the night
following data entry into the computer. After review by the Clinical
Pathologist on service, the reports are sent to the investigator by
campus mail. ALL laboratory data is available for VIEWING with a NYSCVM
MUMPS password using the public function LR. Any error regarding sample
submission should be brought to the attention of the laboratory immediately. |

Protocol changes |
 |
| If there is a change in the original protocol, such as sampling
size, date of submission, etc., please call the laboratory as soon
as possible. Please be aware that the laboratory may not always be
able to accommodate changes to the original protocol. |
