Animal Health Diagnostic Center - Billing & Fee Policies
Client Accounts and Payments
 
 
Payments of all charges are the responsibility of the submitting veterinarian. Charges are billed monthly and we will bill only when all testing on an accession has been finalized.
 
Please remit payments with the top portion of your bill to the following address:

To insure proper management of your account records, your assigned account number must be used in all correspondence with our Laboratory.  Please notify the Billing Department of the Laboratory immediately in writing if there is any change in your address or phone number, if any associates join or leave your practice, or if you dispute charges on your bill.
 
All payments are due upon receipt of your bill.  In order to ensure your payment is reflected on your  next bill, please mail your payment no later than the 25th of the month.  A finance charge will be instituted if payment is not received by the first day of the following month (30 days after initial billing) at the rate of 1% per month, compounded monthly for an effective annual rate of 18% (the minimum finance charge is $.50). Please note: An additional service charge will be assesssed for all checks returned for insufficient funds or similar reasons.
 
The Laboratory accepts Discover Card, Master Card, and Visa for account payments. You may set up a "blanket" charge authorization where we will automatically charge your monthly balance prior to sending your monthly satement.  You may, on the other hand, complete the charge box on your bill each time that you would like to charge your balance.
